Lifestyle Changes To Help Prevent Gallstone Formation

PREVENTION is better than CURE!
LIFESTYLE CHANGES TO HELP PREVENT GALLSTONE FORMATION DIET:  
• Eat whole grains, unrefined carbohydrates (brown bread, rice and pasta etc) and plenty of dietary fibre.
• Increase intake of vegetable proteins (especially soy products), decreasing intake of animal proteins.
• Consume plenty of fresh, organic fruit and vegetables, especially those containing pectin (especially green apples) and cellulose (especially celery and crisp fruits and vegetables).
• Eliminate intake of saturated fats, and ALL products containing white sugar and white flour.
• Eat no fried foods, especially those that are deep-fried.
• Avoid ice-cold foods and drinks - room temperature or slightly warm is best.
• Avoid coffee.
• Drink at least 6-8 glasses of water per day to keep the water content of your bile topped up, plus chamomile tea if possible
• NEVER skip meals and ALWAYS eat breakfast.
• Every meal or snack should contain 10g of fat to enable the gallbladder to contract.
• Drink a little unrefined, cold-pressed extra virgin olive oil each day (see Daily Flush Drink above). Olive oil will help the gallbladder to contract vigorously and thus cleanse itself.
• Do not eat excessively large meals, which put an unnecessary burden on the digestive system.
• Vegetarians have a much lower incidence of gallstones, so adopting a vegetarian diet will certainly help.
Helpful supplements include

2. On the sixth day, take no dinner.

3. At 6 PM, take a teaspoon of Epsom salt (magnesium sulphate) with a glass of warm water.

4. At 8 PM, repeat the same. Magnesium sulphate opens the gallbladder ducts.

5. At 10 PM, take half cup olive oil (or sesame oil) with half cup fresh lemon juice. Mix it well and drink it. The oil lubricates the stones to ease their passage.

The next morning, you will find green stones in your stools. "Usually they float," Chiu Nan notes. "You might want to count them. I have had people who passes 40, 50 or up to 100 stones. Very many."

What To Eat And Avoid When You Have Gallstones

You can’t change your genes, but there are some food and lifestyle factors you can target to lower your risk. Eat healthily, increase your fibre intake, eat more fruit and vegetables high in vitamin C and drink coffee.
Avoid weight gain and if you do try to lose weight, do it slowly.

What does the gallbladder do?

The gall bladder is a storage sac for bile, which is made in liver. After you eat, bile is squirted into the small intestine.

Bile emulsifies or breaks up the fat in the food we eat into tiny particles, a bit like washing-up detergent. This means that fat-digesting enzymes from the pancreas can mix with the fats from foods and break them down into smaller particles that can be absorbed.

What are gallstones?

Gallstones form in the gall bladder when components of bile, such as cholesterol and bilirubin (a breakdown product of red blood cells), aggregate and form stones.
These stones vary in size from single large hard stones about the size of an apricot and made mainly from cholesterol, to small pebble-sized stones made mainly from bilirubin. Gallstones can also be very tiny, like a grainy sludge.

How can you prevent gallstones?

The good news first. Having a healthy diet lowers your risk of getting gallstones.
In a US study that followed more than 13,000 adults over ten years, women with higher blood levels of vitamin C had a lower chance of developing gall bladder disease. This was not the case for men.
Foods high in vitamin C include capsicum, cabbage, Brussels sprouts, cauliflower, broccoli, green leafy vegetables, tomatoes, citrus fruits, pawpaw, kiwi fruit, strawberries and pineapple. You could try vitamin C supplements, but studies are inconsistent and taking vitamin C supplements can increase the risk of kidney stones.
A higher fibre intake is also associated with lower risk. There is no need to become vegetarian, but eating more legumes, pulses, wholegrains, vegetables and fruit boosts fibre intake.

Having a moderate alcohol intake is also associated with a lower risk of gall bladder disease. But it’s wise to drink in line with current guidelines, which recommend no more than two standard drinks on any day.

Rapid weight loss

Losing weight at a rate of more than 1 to 1.5 kilograms a week is considered rapid and increases the risk of gall bladder disease.
When you markedly cut back your total food and fat intake, you need less bile. Your gall bladder doesn’t need to contract as much and bile salts become more concentrated. This means gallstones are more likely to form.
Cutting down on foods that contain large amounts of fat is a good idea because you reduce total kilojoule intake as a result and therefore lose weight. To ensure your gall bladder still contracts and keeps excreting bile, still consume some fat, around 20 grams, each day.

Other risk factors

Risk factors such as body weight, using hormone replacement therapy and number of pregnancies are theoretically modifiable. Others, such as age and being female, are not. Women are twice as likely as men to get gallstones, due to higher oestrogen levels.
Around the world, Pima Indians have the highest rate of gallstones at 70%, compared to 10-30% among Europeans and North Americans and less than 5% in Asian and African populations.

Treatment

Treatment for gall bladder disease depends on size and location of the gallstones, whether you have other symptoms such as pain and infection, and whether you have other medical conditions.
Silent gallstones are ones that cause no symptoms and generally are not treated. Other approaches include the dietary changes mentioned earlier, lithotripsy for small stones, or surgery to remover the gallbladder along with the stones. See your doctor for advice.

What happens after your gall bladder is removed?

Once your gall bladder is removed, bile can no longer be stored, but trickles constantly into the small intestine.
Your digestive tract will make some adjustment after the surgery, but if you eat a large fatty meal, your body is not able to squirt in extra bile. This means some undigested fat passes through the small intestine and travels into the large bowel where bacteria will try to break it down.
However, a lot of the fat will be malabsorbed, triggering steattorhoea, which means fatty diarrhoea. Some fat-soluble vitamin A and E will be lost in the stools as well.
Boost your intake of these vitamins by eating foods that are good sources of them. The vegetable form of vitamin A, called beta-carotene, is found in dark yellow, orange and dark green vegetables, such as pumpkin, carrots, sweet potato, spinach and broccoli.
Good sources of vitamin E include nuts, seeds, wholegrains, peanut butter, tahini, spinach, broccoli, tomato, avocado, kiwifruit and mango.

Gallstones are stones or lumps that develop in the gallbladder or bile duct when certain substances harden.The gallbladder is a small sac located on the right-hand side of the body, on the underside of the liver. Some of the chemicals that exist in the gallbladder can solidify into either one large stone or several small ones.

There are approximately 20 million Americans with gallstones. A study revealed that the prevalence of gallstones in adults in industrialized countries is around 10 percent and appears to be rising.

  • Symptoms

    The majority of people with gallstones experience no symptoms at all. This is because the stones stay in the gallbladder and cause no problems.
    Sometimes, however, gallstones may lead to cholecystitis, or an inflamed gallbladder.
    The primary symptom is pain that comes on suddenly and quickly gets worse. This pain can occur on the right side of the body, just below the ribs, between the shoulder blades, or in the right shoulder.
    Other symptoms include:

    • back pain between the shoulder blades
    • pain in the right shoulder
    • nausea
    • vomiting
    • sweating
    • restlessness
  • Treatment

    Gallstones are only treated if they have caused gallbladderinflammation, blockage of the bile ducts, or if they have moved from the bile ducts into the intestines.
    Cholecystectomy
    Cholecystectomy means the surgical removal of the gallbladder. This is usually performed with keyhole surgery. Keyhole surgery is not possible for about 10 percent of people who need open cholecystectomy. They will have open surgery instead.
    With open cholecystectomy, a large cut is made in the abdomen. People who undergo open surgery require a longer hospital stay and recovery time. If a gallbladder is severely inflamed, open surgery will be needed.
    For a large proportion of those who undergo a cholecystectomy, gallstones come back within a year. To help prevent this, many people with gallstones are given ursosdeoxycholic acid, which is the acid found in bile.
    Ursosdeoxycholic acid lowers the cholesterol content of bile, making it less likely that stones will form.
    Ursodeoxycholic acid
  • If the gallstone is made of cholesterol, it can sometimes be slowly dissolved with ursodeoxycholic acid. This type of treatment, known as dissolution, may take up to 24 months to be effective. It is not as effective as surgery but is sometimes the only choice for people who cannot have a general anesthetic.
    Endoscopic retrograde cholangiopancreatolography
    When a person with gallstones cannot have surgery or ursodeoxycholic acid, they may undergo endoscopic retrograde cholangiopancreatolography (ERCP), which requires a local anesthetic. A flexible fiber-optic camera, or endoscope, goes into mouth, through the digestive system, and into the gallbladder.
    An electrically heated wire widens the opening of the bile duct. The stones are then removed or left to pass into the intestine.
    Lithotripsy
    Ultrasonic shock waves are aimed at the gallstones, which break them up. If gallstones become small enough, they can then pass safely in the stools. This type of treatment is uncommon and is only used when there are few gallstones present.

    Diagnosis

  • In many cases, gallstones are discovered by accident when an individual is being treated for a different condition. A doctor may suspect gallstones after a cholesterol test, an ultrasound scan, a blood test, or even an X-ray.
    Blood tests may be used to look for signs of infection, obstruction, pancreatitis, or jaundice.
    Cholangiography
    A dye is either injected into the blood stream so that it concentrates into the bile ducts or gallbladder, or it is inserted straight into the bile ducts using an ERCP. The dye shows up on X-rays. ERCP is also used to locate and remove stones in the bile duct.
    The doctor will then be able to look at the X-rays and identify possible gallbladder or bile duct disorders, such as pancreatitis, cancer of the pancreas, or gallstones. The X-rays will indicate to the doctor whether the dye is reaching the liver, bile ducts, intestines, and gallbladder.
    If the dye does not move into one of these areas, it generally means that the gallstone is causing a blockage. An expert will have a better idea of where the gallstone is located.
    CT scan
    This is a non-invasive X-ray that produces cross-section pictures of the inside of the human body.
    Cholescintigraphy (HIDA scan)
    A small amount of harmless radioactive material is injected into the patient. This is absorbed by the gallbladder, which is then stimulated to contract. This test may diagnose abnormal contractions of the gallbladder or an obstruction of the bile duct.

    Diet

    It used to be the case that people with gallstones who were not yet ready for surgery would be prescribed an extremely low-fat diet to prevent gallstone growth.
    This has recently been shown to be less helpful than previously thought, as rapid weight loss can cause gallstones.
    A balanced diet with regular meals is recommended. This will not cure gallstones, but it can have a positive impact on any symptoms and pain experienced.
    Avoiding foods high in saturated fats can help to reduce the risk of gallstones developing, such as butter, hard cheese, cakes, and biscuits. Cholesterol is thought to have a role in forming gallstones.
    Dietary steps can be taken to help prevent the condition, such as eating more nuts and consuming small amounts of alcohol.

Gallstones range in size from as small as a grain of sand to as large as a golf ball. Some people develop just one gallstone, while others develop many gallstones at the same time.
People who experience symptoms from their gallstones usually require gallbladder removal surgery. Gallstones that don't cause any signs and symptoms typically don't need treatment.

Symptoms

Gallstones may cause no signs or symptoms. If a gallstone lodges in a duct and causes a blockage, the resulting signs and symptoms may include:
  • Sudden and rapidly intensifying pain in the upper right portion of your abdomen
  • Sudden and rapidly intensifying pain in the center of your abdomen, just below your breastbone
  • Back pain between your shoulder blades
  • Pain in your right shoulder
  • Nausea or vomiting
  • Gallstone pain may last several minutes to a few hours.

    When to see a doctor

    Make an appointment with your doctor if you have any signs or symptoms that worry you.
    Seek immediate care if you develop signs and symptoms of a serious gallstone complication, such as:
    • Abdominal pain so intense that you can't sit still or find a comfortable position
    • Yellowing of your skin and the whites of your eyes
    • High fever with chillsTypes of gallstones
    Types of gallstones that can form in the gallbladder include:
  •  Cholesterol gallstones. The most common type of gallstone, called a cholesterol gallstone, often appears yellow in color. These gallstones are composed mainly of undissolved cholesterol, but may contain other components. 
  • Pigment gallstones. These dark brown or black stones form when your bile contains too much bilirubin.
Causes
It's not clear what causes gallstones to form. Doctors think gallstones may result when:
 Your bile contains too much cholesterol. Normally, your bile contains enough chemicals to dissolve the cholesterol excreted by your liver. But if your liver excretes more cholesterol than your bile can dissolve, the excess cholesterol may form into crystals and eventually into stones.

Your bile contains too much bilirubin. Bilirubin is a chemical that's produced when your body breaks down red blood cells. Certain conditions cause your liver to make too much bilirubin, including liver cirrhosis, biliary tract infections and certain blood disorders. The excess bilirubin contributes to gallstone formation.

Your gallbladder doesn't empty correctly. If your gallbladder doesn't empty completely or often enough, bile may become very concentrated, contributing to the formation of gallstones.
